The template supports common defect types such as crack, corrosion, leak, wear, deformation, overheating, vibration, electrical fault, and other. Each defect entry includes fields for type, description, severity, location, discovery date, and photo.
How is the impact of a defect assessed?
The impact assessment section captures operational impact (none, minor, major, shutdown) and safety impact (none, low, medium, high). Urgency is also rated as immediate, short term, or long term.
Can I report multiple defects in one report?
Yes, the defect details section is repeatable, allowing you to document multiple defects on the same equipment. Each defect entry is independent.
Who signs the defect report?
The report requires signatures from the inspector and the maintenance manager. The inspector documents the defect, and the maintenance manager acknowledges receipt and plans corrective actions.
What recommended actions are available?
Recommended actions include repair, replace, monitor, or further investigation. You can also add a recommended deadline and additional notes.
